To register as a patient of Triad Foot please download the Registration Forms Package by clicking this link and saving them to your computer. Please print these forms and fill in all requested infomation in ink. Completing these forms and bringing or mailing them to our office prior to your first visit will greatly expedite your intake process. The universal Adobe Reader is required to view them. If you don't have Adobe Reader installed you can get it here for either your PC or your Mac. If you do choose to mail this information seal the envelope with tape to provide added security and allow five (5) business days for delivery.
